All nurses are required to recredential, usually every three years, but sometimes more often. The certificate you receive when you first credential will tell you when you need to recredential and you will also receive a reminder email from PapScreen.
To recredential, please complete and submit:
- Recredentialling application form [Word 37K] |
- Self assessment tool [Word 59K] |
- Quality of practice statement for each of your work locations [Word 3.5MB] | [PDF 87K] (if you work in an accredited workplace*)
- Your statistics for the duration of your credentialled period. If you use VCS, you can give permission to have VCS send a copy of these directly to PapScreen. If you do not use VCS, you are required to keep track of your statistics using the PapScreen Statistical worksheet [Excel 976K].
Please note: All nurses need to ask a patient's Aboriginal and Torres Strait Islander status, country of birth and language spoken at home every time they take a Pap test.
Peer supervision sessions
If you have been asked to undertake a peer supervision education session as part of your recredentialling, please ensure you take this Peer Supervisor Education form to your session for the supervising nurse to complete. After your session, this form should be returned to PapScreen ASAP so it can be held on file for when your recredentialling is next due.
If you are no longer practicing as a nurse cervical screening provider, you are required to deactivate your credential status using the Deactivation form [Word 33K] | [PDF 360K]. Please note, you can still remain on the mailing list to receive the Speculum newsletter, and you may reactivate your credential status in the future.
If you have previously been credentialled and wish to reactivate your credential status, please submit the Reactivation form [Word 103K] | [PDF 427K] along with your curriculum vitae to demonstrate your work history since last credentialled. If you have been performing cervical screening elsewhere (e.g. interstate or overseas), it may be useful to submit any statistics you have from this screening.
